Having a baby in Japan: birth & child allowances
Having a child in Japan comes with real financial support, and it’s open to foreign residents. Two benefits do the heavy lifting: a one-off birth lump-sum paid through your health insurance, and a monthly Child Allowance from your municipality. Here’s how each works, from Kyokai Kenpo, the Children and Families Agency, MHLW and the Ministry of Justice.
The birth lump-sum (出産育児一時金)
- Amount: ¥500,000 per child for a birth at 22+ weeks at a facility enrolled in the Japan Obstetric Compensation System (¥488,000 otherwise), for births from April 2023.
- Who gets it: anyone enrolled in Japanese public health insurance — employer-based insurance, Kyokai Kenpo, or municipal National Health Insurance — who gives birth. It also applies from 85 days (about 4 months) of pregnancy, including miscarriage/stillbirth.
- Direct payment: under the chokusetsu shiharai system the hospital claims the allowance straight from your insurer, so at discharge you generally pay only the difference if the bill exceeds the allowance (and can claim a refund if it’s less).
Child Allowance (児童手当, jido teate)
A monthly benefit paid by your municipality:

It runs from birth through the first 31 March after the child turns 18, and is paid six times a year in even months. Since the October 2024 reform there is no income limit. Crucially, it is not automatic — you file an application (認定請求書) at your city/ward office, ideally within about 15 days of the birth, to avoid missing a payment month. An official municipal FAQ confirms foreign residents registered in the city can be the applicant, as long as the child is also registered as a resident of Japan.
First things first: register the birth
A birth in Japan must be registered with a Notification of Birth (出生届) within 14 days, at the municipal office for the birthplace, the family’s registered domicile, or the notifier’s residence. It can be filed by the father, mother, a co-resident, or the attending doctor/midwife (Ministry of Justice). Doing this promptly also feeds the insurance and allowance steps above.
